Ensure you have all the necessary documents ready before enrollment.
For Incoming First Year
- Enrollment Form
- Form 138 (High School Report Card)
- ood Moral Certificate
- (4) pcs. photocopy of PSA-Issued Birth Certificates
- (4) pcs. 2x2 or* passport size I'D pictures
- Barangay Certification of Indigency/ Low Income
- Medical Certificate
- Drug Test (for Criminology*)
For Transferees
- Certificate of Transfer Credentials (CTC) or Certificate of Honorable Dismissal
- Transcript of Records
- ertificate of Good Moral Character
- (4) pcs photocopy of PSA Birth Certificates
- (4) pcs. 2x2 or* passport size I'D pictures
- Barangay Certification of Indigency/Low Income
Reminder: All enrollment requirements for College must be submitted in a long envelope with color based in the respective department.
For Incoming Grade 7 and 11
- Form 138 (Report Card)
- Good Moral Certificate
- PSA-Issued Birth Certificate
- (4) pcs. 2x2 I'D pictures
- A&E Assessment and Certification (if ALS)
- Notarized Certificate of Low Income (Parents and Guardian)
- Green envelope
